Our affiliate program is powered by JVManager, you can find the instructions for affiliates at http://www.jvmanager.com/affiliates/help .

You don't have to fill in all the information in below form. If you don't want to provide the information now, please just fill in 'N/A', and you can always change it later.

* For each sale, the commission for affiliate is $8 for each sale.
* The commission will be sent out 30 days after the sales were made.
* Nobody should use his own affiliate link to order the product. We will NOT pay commission if you order the product using your own affiliate link.
