You don't have to fill in all the information in below form. If you don't want to provide the information now, please just fill in 'N/A', and you can always change it later.

* For each sale, the commission for affiliate is = (the sale's price - payment processing fee) * 40%. The payment processing fee is $1 for monthly subscription or $9 for one-time fee order. So, for each monthly subscription, the commission will be $9.6 and for one-time fee order, the commission will be $96. If it is a subscription, the commission will be sent out each month.
* The commission will be sent out 30 days after the sales were made.
* Nobody should use his own affiliate link to order the product. We will NOT pay commission if you order the product using your own affiliate link.
